This makes the includes more explicit, and is preparation for moving
md_k.h to drivers/md/md.h

Remove include/raid/md.h as its only remaining use was to #include
other files.

When the total length is shorter than the calculated number of unaligned bytes, the call to shash-&gt;update breaks. For example, calling crc32c on unaligned buffer with length of 1 can result in a system crash.

To allow an async_tx routine to be compiled away on HAS_DMA=n arch it
needs to be declared __always_inline otherwise the compiler may emit
code and cause a link error.
