__split_huge_pmd_locked() can be called for a present THP, devmap or
(non-present) migration entry. It calls pmdp_invalidate() unconditionally
on the pmdp and only determines if it is present or not based on the
returned old pmd. This is a problem for the migration entry case because
pmd_mkinvalid(), called by pmdp_invalidate() must only be called for a
present pmd.
On arm64 at least, pmd_mkinvalid() will mark the pmd such that any future
call to pmd_present() will return true. And therefore any lockless
pgtable walker could see the migration entry pmd in this state and start
interpretting the fields as if it were present, leading to BadThings (TM).
GUP-fast appears to be one such lockless pgtable walker.
x86 does not suffer the above problem, but instead pmd_mkinvalid() will
corrupt the offset field of the swap entry within the swap pte. See link
below for discussion of that problem.
Fix all of this by only calling pmdp_invalidate() for a present pmd. And
for good measure let's add a warning to all implementations of
pmdp_invalidate[_ad](). I've manually reviewed all other
pmdp_invalidate[_ad]() call sites and believe all others to be conformant.
This is a theoretical bug found during code review. I don't have any test
case to trigger it in practice.

Add the generic pte_free_defer(), to call pte_free() via call_rcu().
pte_free_defer() will be called inside khugepaged's retract_page_tables()
loop, where allocating extra memory cannot be relied upon. This version
suits all those architectures which use an unfragmented page for one page
table (none of whose pte_free()s use the mm arg which was passed to it).

There is a faint risk that __pte_offset_map(), on a 32-bit architecture
with a 64-bit pmd_t e.g. x86-32 with CONFIG_X86_PAE=y, would succeed on a
pmdval assembled from a pmd_low and a pmd_high which never belonged
together: their combination not pointing to a page table at all, perhaps
not even a valid pfn. pmdp_get_lockless() is not enough to prevent that.
Guard against that (on such configs) by local_irq_save() blocking TLB
flush between present updates, as linux/pgtable.h suggests. It's only
needed around the pmdp_get_lockless() in __pte_offset_map(): a race when
__pte_offset_map_lock() repeats the pmdp_get_lockless() after getting the
lock, would just send it back to __pte_offset_map() again.
Complement this pmdp_get_lockless_start() and pmdp_get_lockless_end(),
used only locally in __pte_offset_map(), with a pmdp_get_lockless_sync()
synonym for tlb_remove_table_sync_one(): to send the necessary interrupt
at the right moment on those configs which do not already send it.
CONFIG_GUP_GET_PXX_LOW_HIGH is enabled when required by mips, sh and x86.
It is not enabled by arm-32 CONFIG_ARM_LPAE: my understanding is that Will
Deacon's 2020 enhancements to READ_ONCE() are sufficient for arm. It is
not enabled by arc, but its pmd_t is 32-bit even when pte_t 64-bit.
Limit the IRQ disablement to CONFIG_HIGHPTE? Perhaps, but would need a
little more work, to retry if pmd_low good for page table, but pmd_high
non-zero from THP (and that might be making x86-specific assumptions).

Patch series "mm: free retracted page table by RCU", v3.
Some mmap_lock avoidance i.e. latency reduction. Initially just for the
case of collapsing shmem or file pages to THPs: the usefulness of
MADV_COLLAPSE on shmem is being limited by that mmap_write_lock it
currently requires.
Likely to be relied upon later in other contexts e.g. freeing of empty
page tables (but that's not work I'm doing). mmap_write_lock avoidance
when collapsing to anon THPs? Perhaps, but again that's not work I've
done: a quick attempt was not as easy as the shmem/file case.
These changes (though of course not these exact patches) have been in
Google's data centre kernel for three years now: we do rely upon them.
This patch (of 13):
Before putting them to use (several commits later), add rcu_read_lock() to
pte_offset_map(), and rcu_read_unlock() to pte_unmap(). Make this a
separate commit, since it risks exposing imbalances: prior commits have
fixed all the known imbalances, but we may find some have been missed.

Convert all instances of direct pte_t* dereferencing to instead use
ptep_get() helper. This means that by default, the accesses change from a
C dereference to a READ_ONCE(). This is technically the correct thing to
do since where pgtables are modified by HW (for access/dirty) they are
volatile and therefore we should always ensure READ_ONCE() semantics.
But more importantly, by always using the helper, it can be overridden by
the architecture to fully encapsulate the contents of the pte. Arch code
is deliberately not converted, as the arch code knows best. It is
intended that arch code (arm64) will override the default with its own
implementation that can (e.g.) hide certain bits from the core code, or
determine young/dirty status by mixing in state from another source.
Conversion was done using Coccinelle:

Then reviewed and hand-edited to avoid multiple unnecessary calls to
ptep_get(), instead opting to store the result of a single call in a
variable, where it is correct to do so. This aims to negate any cost of
READ_ONCE() and will benefit arch-overrides that may be more complex.
Included is a fix for an issue in an earlier version of this patch that
was pointed out by kernel test robot. The issue arose because config
MMU=n elides definition of the ptep helper functions, including
ptep_get(). HUGETLB_PAGE=n configs still define a simple
huge_ptep_clear_flush() for linking purposes, which dereferences the ptep.
So when both configs are disabled, this caused a build error because
ptep_get() is not defined. Fix by continuing to do a direct dereference
when MMU=n. This is safe because for this config the arch code cannot be
trying to virtualize the ptes because none of the ptep helpers are
defined.

Make pte_offset_map() a wrapper for __pte_offset_map() (optionally outputs
pmdval), pte_offset_map_lock() a sparse __cond_lock wrapper for
__pte_offset_map_lock(): those __funcs added in mm/pgtable-generic.c.
__pte_offset_map() do pmdval validation (including pmd_clear_bad() when
pmd_bad()), returning NULL if pmdval is not for a page table.
__pte_offset_map_lock() verify pmdval unchanged after getting the lock,
trying again if it changed.
No #ifdef CONFIG_TRANSPARENT_HUGEPAGE around them: that could be done to
cover the imminent case, but we expect to generalize it later, and it
makes a mess of where to do the pmd_bad() clearing.
Add pte_offset_map_nolock(): outputs ptl like pte_offset_map_lock(),
without actually taking the lock. This will be preferred to open uses of
pte_lockptr(), because (when split ptlock is in page table's struct page)
it points to the right lock for the returned pte pointer, even if *pmd
gets changed racily afterwards.
Update corresponding Documentation.
Do not add the anticipated rcu_read_lock() and rcu_read_unlock()s yet:
they have to wait until all architectures are balancing pte_offset_map()s
with pte_unmap()s (as in the arch series posted earlier). But comment
where they will go, so that it's easy to add them for experiments. And
only when those are in place can transient racy failure cases be enabled.
Add more safety for the PAE mismatched pmd_low pmd_high case at that time.

s390 can do more fine-grained handling of spurious TLB protection faults,
when there also is the PTE pointer available.
Therefore, pass on the PTE pointer to flush_tlb_fix_spurious_fault() as an
additional parameter.
This will add no functional change to other architectures, but those with
private flush_tlb_fix_spurious_fault() implementations need to be made
aware of the new parameter.

Calls to change_protection_range() on THP can trigger, at least on x86,
two TLB flushes for one page: one immediately, when pmdp_invalidate() is
called by change_huge_pmd(), and then another one later (that can be
batched) when change_protection_range() finishes.
The first TLB flush is only necessary to prevent the dirty bit (and with a
lesser importance the access bit) from changing while the PTE is modified.
However, this is not necessary as the x86 CPUs set the dirty-bit
atomically with an additional check that the PTE is (still) present. One
caveat is Intel's Knights Landing that has a bug and does not do so.
Leverage this behavior to eliminate the unnecessary TLB flush in
change_huge_pmd(). Introduce a new arch specific pmdp_invalidate_ad()
that only invalidates the access and dirty bit from further changes.

linux/mm_types.h should only define structure definitions, to make it
cheap to include elsewhere. The atomic_t helper function definitions
are particularly large, so it's better to move the helpers using those
into the existing linux/mm_inline.h and only include that where needed.
As a follow-up, we may want to go through all the indirect includes in
mm_types.h and reduce them as much as possible.

Patch series "mm/thp: fix THP splitting unmap BUGs and related", v10.
Here is v2 batch of long-standing THP bug fixes that I had not got
around to sending before, but prompted now by Wang Yugui's report

Wang Yugui has tested a rollup of these fixes applied to 5.10.39, and
they have done no harm, but have *not* fixed that issue: something more
is needed and I have no idea of what.
This patch (of 7):
Stressing huge tmpfs page migration racing hole punch often crashed on
the VM_BUG_ON(!pmd_present) in pmdp_huge_clear_flush(), with DEBUG_VM=y
kernel; or shortly afterwards, on a bad dereference in
__split_huge_pmd_locked() when DEBUG_VM=n. They forgot to allow for pmd
migration entries in the non-anonymous case.
Full disclosure: those particular experiments were on a kernel with more
relaxed mmap_lock and i_mmap_rwsem locking, and were not repeated on the
vanilla kernel: it is conceivable that stricter locking happens to avoid
those cases, or makes them less likely; but __split_huge_pmd_locked()
already allowed for pmd migration entries when handling anonymous THPs,
so this commit brings the shmem and file THP handling into line.
And while there: use old_pmd rather than _pmd, as in the following
blocks; and make it clearer to the eye that the !vma_is_anonymous()
block is self-contained, making an early return after accounting for
unmapping.

pmdp_huge_clear_flush()
The developer will have trouble figuring out why the BUG actually
triggered when there is a complex expression in the VM_BUG_ON. Because we
can only identify the condition triggered BUG via line number provided by
VM_BUG_ON. Optimize this by spliting such a complex expression into two
simple conditions.

pmd_present() is expected to test positive after pmdp_mknotpresent() as
the PMD entry still points to a valid huge page in memory.
pmdp_mknotpresent() implies that given PMD entry is just invalidated from
MMU perspective while still holding on to pmd_page() referred valid huge
page thus also clearing pmd_present() test. This creates the following
situation which is counter intuitive.
[pmd_present(pmd_mknotpresent(pmd)) = true]
This renames pmd_mknotpresent() as pmd_mkinvalid() reflecting the helper's
functionality more accurately while changing the above mentioned situation
as follows. This does not create any functional change.
[pmd_present(pmd_mkinvalid(pmd)) = true]
This is not applicable for platforms that define own pmdp_invalidate() via
__HAVE_ARCH_PMDP_INVALIDATE. Suggestion for renaming came during a
previous discussion here.

Vlastimil noted that pmdp_invalidate() is not atomic and we can lose
dirty and access bits if CPU sets them after pmdp dereference, but
before set_pmd_at().
The patch change pmdp_invalidate() to make the entry non-present
atomically and return previous value of the entry. This value can be
used to check if CPU set dirty/accessed bits under us.
The race window is very small and I haven't seen any reports that can be
attributed to the bug. For this reason, I don't think backporting to
stable trees needed.

Many source files in the tree are missing licensing information, which
makes it harder for compliance tools to determine the correct license.
By default all files without license information are under the default
license of the kernel, which is GPL version 2.
Update the files which contain no license information with the 'GPL-2.0'
SPDX license identifier. The SPDX identifier is a legally binding
shorthand, which can be used instead of the full boiler plate text.
This patch is based on work done by Thomas Gleixner and Kate Stewart and
Philippe Ombredanne.
How this work was done:
Patches were generated and checked against linux-4.14-rc6 for a subset of
the use cases:
- file had no licensing information it it.
- file was a */uapi/* one with no licensing information in it,
- file was a */uapi/* one with existing licensing information,
Further patches will be generated in subsequent months to fix up cases
where non-standard license headers were used, and references to license
had to be inferred by heuristics based on keywords.
The analysis to determine which SPDX License Identifier to be applied to
a file was done in a spreadsheet of side by side results from of the
output of two independent scanners (ScanCode & Windriver) producing SPDX
tag:value files created by Philippe Ombredanne. Philippe prepared the
base worksheet, and did an initial spot review of a few 1000 files.
The 4.13 kernel was the starting point of the analysis with 60,537 files
assessed. Kate Stewart did a file by file comparison of the scanner
results in the spreadsheet to determine which SPDX license identifier(s)
to be applied to the file. She confirmed any determination that was not
immediately clear with lawyers working with the Linux Foundation.
Criteria used to select files for SPDX license identifier tagging was:
- Files considered eligible had to be source code files.
- Make and config files were included as candidates if they contained >5
lines of source
- File already had some variant of a license header in it (even if <5
lines).
All documentation files were explicitly excluded.
The following heuristics were used to determine which SPDX license
identifiers to apply.
- when both scanners couldn't find any license traces, file was
considered to have no license information in it, and the top level
COPYING file license applied.
For non */uapi/* files that summary was:

- when the two scanners agreed on the detected license(s), that became
the concluded license(s).
- when there was disagreement between the two scanners (one detected a
license but the other didn't, or they both detected different
licenses) a manual inspection of the file occurred.
- In most cases a manual inspection of the information in the file
resulted in a clear resolution of the license that should apply (and
which scanner probably needed to revisit its heuristics).
- When it was not immediately clear, the license identifier was
confirmed with lawyers working with the Linux Foundation.
- If there was any question as to the appropriate license identifier,
the file was flagged for further research and to be revisited later
in time.
In total, over 70 hours of logged manual review was done on the
spreadsheet to determine the SPDX license identifiers to apply to the
source files by Kate, Philippe, Thomas and, in some cases, confirmation
by lawyers working with the Linux Foundation.
Kate also obtained a third independent scan of the 4.13 code base from
FOSSology, and compared selected files where the other two scanners
disagreed against that SPDX file, to see if there was new insights. The
Windriver scanner is based on an older version of FOSSology in part, so
they are related.
Thomas did random spot checks in about 500 files from the spreadsheets
for the uapi headers and agreed with SPDX license identifier in the
files he inspected. For the non-uapi files Thomas did random spot checks
in about 15000 files.
In initial set of patches against 4.14-rc6, 3 files were found to have
copy/paste license identifier errors, and have been fixed to reflect the
correct identifier.
Additionally Philippe spent 10 hours this week doing a detailed manual
inspection and review of the 12,461 patched files from the initial patch
version early this week with:
- a full scancode scan run, collecting the matched texts, detected
license ids and scores
- reviewing anything where there was a license detected (about 500+
files) to ensure that the applied SPDX license was correct
- reviewing anything where there was no detection but the patch license
was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied
SPDX license was correct
This produced a worksheet with 20 files needing minor correction. This
worksheet was then exported into 3 different .csv files for the
different types of files to be modified.
These .csv files were then reviewed by Greg. Thomas wrote a script to
parse the csv files and add the proper SPDX tag to the file, in the
format that the file expected. This script was further refined by Greg
based on the output to detect more types of files automatically and to
distinguish between header and source .c files (which need different
comment types.) Finally Greg ran the script using the .csv files to
generate the patches.

Add thp migration's core code, including conversions between a PMD entry
and a swap entry, setting PMD migration entry, removing PMD migration
entry, and waiting on PMD migration entries.
This patch makes it possible to support thp migration. If you fail to
allocate a destination page as a thp, you just split the source thp as
we do now, and then enter the normal page migration. If you succeed to
allocate destination thp, you enter thp migration. Subsequent patches
actually enable thp migration for each caller of page migration by
allowing its get_new_page() callback to allocate thps.

The current transparent hugepage code only supports PMDs. This patch
adds support for transparent use of PUDs with DAX. It does not include
support for anonymous pages. x86 support code also added.
Most of this patch simply parallels the work that was done for huge
PMDs. The only major difference is how the new ->pud_entry method in
mm_walk works. The ->pmd_entry method replaces the ->pte_entry method,
whereas the ->pud_entry method works along with either ->pmd_entry or
->pte_entry. The pagewalk code takes care of locking the PUD before
calling ->pud_walk, so handlers do not need to worry whether the PUD is
stable.

We remove one instace of flush_tlb_range here. That was added by commit
f714f4f20e59 ("mm: numa: call MMU notifiers on THP migration"). But the
pmdp_huge_clear_flush_notify should have done the require flush for us.
Hence remove the extra flush.

This showed up on ARC when running LMBench bw_mem tests as Overlapping
TLB Machine Check Exception triggered due to STLB entry (2M pages)
overlapping some NTLB entry (regular 8K page).
bw_mem 2m touches a large chunk of vaddr creating NTLB entries. In the
interim khugepaged kicks in, collapsing the contiguous ptes into a
single pmd. pmdp_collapse_flush()->flush_pmd_tlb_range() is called to
flush out NTLB entries for the ptes. This for ARC (by design) can only
shootdown STLB entries (for pmd). The stray NTLB entries cause the
overlap with the subsequent STLB entry for collapsed page. So make
pmdp_collapse_flush() call pte flush interface not pmd flush.
Note that originally all thp flush call sites in generic code called
flush_tlb_range() leaving it to architecture to implement the flush for
pte and/or pmd. Commit 12ebc1581ad11454 changed this by calling a new
opt-in API flush_pmd_tlb_range() which made the semantics more explicit
but failed to distinguish the pte vs pmd flush in generic code, which is
what this patch fixes.
Note that ARC can fixed w/o touching the generic pmdp_collapse_flush()
by defining a ARC version, but that defeats the purpose of generic
version, plus sementically this is the right thing to do.

A dax-huge-page mapping while it uses some thp helpers is ultimately not
a transparent huge page. The distinction is especially important in the
get_user_pages() path. pmd_devmap() is used to distinguish dax-pmds
from pmd_huge() and pmd_trans_huge() which have slightly different
semantics.
Explicitly mark the pmd_trans_huge() helpers that dax needs by adding
pmd_devmap() checks.

ARCHes with special requirements for evicting THP backing TLB entries
can implement this.
Otherwise also, it can help optimize TLB flush in THP regime.
stock flush_tlb_range() typically has optimization to nuke the entire
TLB if flush span is greater than a certain threshhold, which will
likely be true for a single huge page. Thus a single thp flush will
invalidate the entrire TLB which is not desirable.

We have confusing functions to clear pmd, pmd_clear_* and pmd_clear. Add
_huge_ to pmdp_clear functions so that we are clear that they operate on
hugepage pte.
We don't bother about other functions like pmdp_set_wrprotect,
pmdp_clear_flush_young, because they operate on PTE bits and hence
indicate they are operating on hugepage ptes

There are a few subtle races, between change_protection_range (used by
mprotect and change_prot_numa) on one side, and NUMA page migration and
compaction on the other side.
The basic race is that there is a time window between when the PTE gets
made non-present (PROT_NONE or NUMA), and the TLB is flushed.
During that time, a CPU may continue writing to the page.
This is fine most of the time, however compaction or the NUMA migration
code may come in, and migrate the page away.
When that happens, the CPU may continue writing, through the cached
translation, to what is no longer the current memory location of the
process.
This only affects x86, which has a somewhat optimistic pte_accessible.
All other architectures appear to be safe, and will either always flush,
or flush whenever there is a valid mapping, even with no permissions
(SPARC).
The basic race looks like this:

make entry PTE/PMD_NUMA
fault on entry
read/write old page
start migrating page
change PTE/PMD to new page
read/write old page [*]
flush TLB
reload TLB from new entry
read/write new page
lose data
[*] the old page may belong to a new user at this point!
The obvious fix is to flush remote TLB entries, by making sure that
pte_accessible aware of the fact that PROT_NONE and PROT_NUMA memory may
still be accessible if there is a TLB flush pending for the mm.
This should fix both NUMA migration and compaction.

On x86, PMD entries are similar to _PAGE_PROTNONE protection and are
handled as NUMA hinting faults. The following two page table protection
bits are what defines them
_PAGE_NUMA:set _PAGE_PRESENT:clear
A PMD is considered present if any of the _PAGE_PRESENT, _PAGE_PROTNONE,
_PAGE_PSE or _PAGE_NUMA bits are set. If pmdp_invalidate encounters a
pmd_numa, it clears the present bit leaving _PAGE_NUMA which will be
considered not present by the CPU but present by pmd_present. The
existing caller of pmdp_invalidate should handle it but it's an
inconsistent state for a PMD. This patch keeps the state consistent
when calling pmdp_invalidate.

Currently mm->pmd_huge_pte protected by page table lock. It will not
work with split lock. We have to have per-pmd pmd_huge_pte for proper
access serialization.
For now, let's just introduce wrapper to access mm->pmd_huge_pte.

accessible anyway by the CPU, eg. a _PAGE_PROTNONE page table entry,
there is no need to flush the TLB on remote CPUs.

The function ptep_set_access_flags is only ever used to upgrade
access permissions to a page. That means the only negative side
effect of not flushing remote TLBs is that other CPUs may incur
spurious page faults, if they happen to access the same address,
and still have a PTE with the old permissions cached in their
TLB.
Having another CPU maybe incur a spurious page fault is faster
than always incurring the cost of a remote TLB flush, so replace
the remote TLB flush with a purely local one.
This should be safe on every architecture that correctly
implements flush_tlb_fix_spurious_fault() to actually invalidate
the local TLB entry that caused a page fault, as well as on
architectures where the hardware invalidates TLB entries that
cause page faults.
In the unlikely event that you are hitting what appears to be
an infinite loop of page faults, and 'git bisect' took you to
this changeset, your architecture needs to implement
flush_tlb_fix_spurious_fault to actually flush the TLB entry.

On s390, a valid page table entry must not be changed while it is attached
to any CPU. So instead of pmd_mknotpresent() and set_pmd_at(), an IDTE
operation would be necessary there. This patch introduces the
pmdp_invalidate() function, to allow architecture-specific
implementations.

The thp page table pre-allocation code currently assumes that pgtable_t is
of type "struct page *". This may not be true for all architectures, so
this patch removes that assumption by replacing the functions
prepare_pmd_huge_pte() and get_pmd_huge_pte() with two new functions that
can be defined architecture-specific.
It also removes two VM_BUG_ON checks for page_count() and page_mapcount()
operating on a pgtable_t. Apart from the VM_BUG_ON removal, there will be
no functional change introduced by this patch.

The change adds some infrastructure for managing tile pmd's more generally,
using pte_pmd() and pmd_pte() methods to translate pmd values to and
from ptes, since on TILEPro a pmd is really just a nested structure
holding a pgd (aka pte). Several existing pmd methods are moved into
this framework, and a whole raft of additional pmd accessors are defined
that are used by the transparent hugepage framework.
The tile PTE now has a "client2" bit. The bit is used to indicate a
transparent huge page is in the process of being split into subpages.
This change also fixes a generic bug where the return value of the
generic pmdp_splitting_flush() was incorrect.

These macros will be used in a later patch, where all usages are expected
to be optimized away without #ifdef CONFIG_TRANSPARENT_HUGEPAGE. But to
detect unexpected usages, we convert the existing BUG() to BUILD_BUG().

pmdp_get_and_clear/pmdp_clear_flush/pmdp_splitting_flush were trapped as
BUG() and they were defined only to diminish the risk of build issues on
not-x86 archs and to be consistent with the generic pte methods previously
defined in include/asm-generic/pgtable.h.
But they are causing more trouble than they were supposed to solve, so
it's simpler not to define them when THP is off.
This is also correcting the export of pmdp_splitting_flush which is
currently unused (x86 isn't using the generic implementation in
mm/pgtable-generic.c and no other arch needs that [yet]).
